AZ Super Bowl Quiz Game
NO PURCHASE NECESSARY TO ENTER OR CLAIM PRIZE. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. ENTRANTS MUST BE 18 YEARS OF AGE OR OLDER TO BE ELIGIBLE TO WIN. VOID WHERE PROHIBITED BY LAW. THIS QUIZ GAME MAY NOT BE USED TO CONDUCT, ADVERTISE, OR PROMOTE ANY FORM OF GAMBLING.
The Arizona Super Bowl Host Committee (“Committee”) in conjunction with Dignity Health – Arizona Service Area, 3030 N. Central Ave., Suite 1107, Phoenix, AZ 85012 (collectively, the “Sponsors”) is pleased to offer the AZ Super Bowl Quiz Game (“Quiz”) as administered on the QuizWizards’ Quiz platform (“Quiz Administrator”). Sponsors and Quiz Administrator will be collectively referred to as the Quiz Promotion Entities (“Quiz Promotion Entities”). As a condition of participating in the Quiz, you agree to be fully and unconditionally bound by these official rules (“Official Rules”) and the decisions of Sponsors, whose decisions shall be final and binding in all respects, and to waive any right to claim ambiguity in the Quiz, these Official Rules, or any materials, websites or advertising associated with the Quiz. The Quiz is subject to applicable federal, state, provincial and local laws and regulations and is VOID WHERE PROHIBITED OR RESTRICTED BY LAW. Winning and receiving a Prize (as defined in Section 6) is contingent upon fulfilling all requirements set forth herein.
The Quiz will be comprised of three (3) distinct quiz periods (each, a “Quiz Period” and collectively, the “Quiz Periods”). The first Quiz Period begins at 7:00 AM Mountain Standard Time (“MT”) on January 20, 2023 and ends at 7:00 PM MT on January 25, 2023 (“First Quiz Period”). The second Quiz Period begins at 7:00 AM MT on January 26, 2023 and ends at 7:00 PM MT on February 1, 2023 (“Second Quiz Period”). The third Quiz Period begins at 7:00 AM MT on February 2, 2023 and ends at 7:00 PM MT on February 8, 2023 (“Third Quiz Period”). Sponsor’s clock shall be the official clock for purposes of each Quiz Period.
In order to be eligible to participate in the Quiz, you must: (i) be a legal resident of the state of Arizona who is 13 years of age or older as of the date of entry, (ii) not be a person barred from participation in the Quiz; and (iii) at all times abide by these Official Rules. Any individuals (including but not limited to employees, consultants, independent contractors, and interns) who have, within the past six (6) months, performed services for the Committee, or the National Football League (“NFL”), or any organization responsible for sponsoring, fulfilling, or administering the Quiz, or supplying the Prize, or who won any Quiz or Prize sponsored by Committee, or any of their respective parents, subsidiaries, affiliates, or successor companies, and immediate family and household members thereof, are not eligible to enter. “Immediate family members” means parents, stepparents, children, stepchildren, siblings, stepsiblings, or spouses. “Household members” shall mean people who share the same residence at least three (3) months in the preceding one (1) calendar year.
NO PURCHASE NECESSARY TO ENTER. Eligible users may enter the Quiz once per Quiz Period, for a total of three (3) total entries.
During the respective Quiz Periods, eligible users must visit the Committee’s website, www.azsuperbowl.com/quiz, and click PLAY NOW to take part in the Quiz. Each Quiz during the applicable Quiz Period will consist of ten (10) questions. Each Quiz will consist of the same questions for each Quiz Period. All questions are multiple choice with four (4) possible answers, with only one (1) correct answer. There will be a ten (10) second timer for each question, before the next question is presented. Each correct answer is worth an initial one thousand (1,000) points. However, for every millisecond taken to answer, the total amount of points able to be awarded are reduced by one (1) point. There is no penalty for answering incorrectly or failing to answer a question. Once an answer is selected, such selection will turn green if answered correctly, or red if answered incorrectly.
Once eligible users have completed a Quiz, they must accurately complete and successfully submit their name, email and year of birth on the Quiz entry form in order to submit their answers, see their position on the Quiz leaderboard and be eligible of winning a Prize. Users must fully complete and submit all non-optional data requested on the Quiz entry form to be eligible. Incomplete and/or inaccurate entries are void. Entrants in any Quiz may be given the option to receive commercial emails from the Committee; however, eligibility to participate in any Quiz is not dependent upon entrant’s consent to receive such emails and updates and consenting to receive such emails and updates will not impact any entrant’s chances of winning.
No other method of entry will be accepted. Limit one (1) entry per person, per Quiz Period. Sponsors reserve the right to void all entries for any person that submits additional entries beyond the specified limit at its sole and absolute discretion. Using an automated system to participate in the Quiz or attempting to subvert the Quiz entry process is prohibited and may result in disqualification. In the event of a dispute over who submitted a winning entry, the Authorized Account Holder for the email address associated with the entry will be deemed to be the entrant. The Authorized Account Holder is defined as the natural person that has established the corresponding account with the email provider.
Failure to follow any instructions provided by Sponsors or failure to submit your entry within the applicable Quiz Periods may result in disqualification. The time of receipt of any entry shall be the time such valid entry becomes available to Sponsors. The use of script, macro, other automated means or any other device to subvert the registration process is prohibited and all such registrations will be void. The Committee as well as the other Quiz Promotion Entities are not responsible for incorrect or inaccurate transcription of entry information, computer, online or human error, technical malfunctions, any omission, interruption, defect, or any other error, including, without limitation, lost, late, incomplete, invalid, unintelligible or misdirected entries, all of which may constitute grounds for disqualification in Sponsors’ sole and absolute discretion. If a problem occurs in the entry process, the entrant is solely responsible for verifying that the entry process has been validly completed. Sponsors have no obligation to advise an entrant of an incomplete, late or undeliverable entry. All entries become the sole and exclusive property of Sponsors and will not be acknowledged or returned. Entrant is responsible for all online charges incurred with any Internet service provider and/or wireless carrier.
There will be three (3) first-place Prize winners, one (1) per Quiz Period (“First-Place Winner”), and three (3) runner-up Prize winners (“Runner-Ups”), again one (1) per Quiz Period. Each First-Place Winner as well Runner-Ups (collectively, the “Winners”) are only potential Winners until verification of eligibility requirements is complete.
The eligible user with the highest score on the leaderboard at the end each Quiz Period will be the potential First-Place Winner for that Quiz Period. The eligible user with the second highest score on the leaderboards at the end of each Quiz Period will be the Runner-Up for that Quiz Period. If there is a tie on the leaderboard, the eligible user who first reached that score will be deemed the potential First-Place Winner or Runner-Up, respectively. If an eligible user plays the Quiz a second or subsequent time during a Quiz Period, using the same email address or a different email address, only the first attempt will be counted as all subsequent entries will be void.
Sponsors will have sole and absolute discretion over interpretation of the Official Rules, the administration of the Quiz, and the determination of the winners. Decisions of Sponsors as to the determination of the Winners will be final.
The odds of winning the Prize depends on the total number of eligible users entering during the Quiz Periods and their respective trivia skills.
Sponsors will attempt to notify potential Winners by email within 48 hours of the conclusion of the respective Quiz Period. Inability of Sponsors to contact a potential Winner, or to receive a response from a potential Winner within 48 hours of the conclusion of a Quiz Period may result in the Prize being forfeited. If a Prize is forfeited, the eligible user with the next highest score in each Quiz Period may be deemed the respective First-Place Winner or Runner-Up, as applicable, and at the Sponsors’ sole and absolute discretion.
Sponsors may also deliver via email to the potential Winners an Affidavit of Eligibility and Liability and a Publicity Release (“Release”) to be executed and returned via email (or by facsimile) within 48 hours after receipt of the email. The Release (i) indemnifies and holds harmless the Sponsors and any Released Parties (as defined in Section 8) from and against any and all harm, expense, loss, damage, liability, or injury resulting directly or indirectly from submitting an entry in the Quiz or awarding, receipt, possession, use, or misuse of any Prize awarded in the Quiz, (ii) contains the potential Winner’s agreement to be bound by the Quiz’s Official Rules, and (iii) authorizes the Committee to use the potential Winners’ names, likenesses, photographs, voices, and biographical data for advertising, promotional, and any other purpose, in any media.
All potential Winners are subject to verification of eligibility requirements. If a potential Winner does not meet Official Rules or eligibility requirements, fails to complete, sign and return the applicable Release within the required time period, or if the Release is returned as undeliverable or incomplete, the potential Winner p may be disqualified at Sponsors’ sole and absolute discretion. In the event a potential Winner is disqualified for any reason or if the Prize is unclaimed, the Prize will be forfeited. Any forfeited Prize may be awarded to an alternate winner, at the Sponsors’ sole and absolute discretion. Limit one Prize per person and per household.
ALL POTENTIAL WINNERS AND POTENTIAL RUNNER-UPS ARE SUBJECT TO VERIFICATION BY SPONSOR. SPONSOR’S DECISIONS AS TO THE ADMINISTRATION AND OPERATION OF THE QUIZ, ANY CHANGES TO THESE OFFICIAL RULES, AND THE SELECTION OF POTENTIAL WINNERS IS FINAL AND BINDING IN ALL MATTERS RELATED TO THE QUIZ.
Subject to verification of eligibility and compliance with these Official Rules, six (6) prizes will be awarded (each individually, a “Prize” and collectively, the “Prizes). The Prizes and their approximate retail values (“ARV”) are as follows:
The ARV for all Prizes is $ 600 USD. The ARV for a Prize is an estimate made before the Quiz begins and may vary. If the actual retail value of the Prize is less than or greater than the stated ARV, the Prize winner will not receive the difference.
All Prizes won will be mailed out at the address provided to Sponsors upon winning the Quiz. Limit one (1) Prize per person and per household. No transfer, substitution or cash equivalent for a Prize will be made, except that the Committee, at its sole and absolute discretion, may substitute a Prize (or component of a Prize) of equal or greater value in the event the above Prizes are unavailable for any reason. Any depiction of the Prize, in Quiz materials or otherwise, is for illustrative purposes only. ALL FEDERAL, STATE AND LOCAL TAXES ARE THE SOLE RESPONSIBILITY OF THE PRIZE WINNERS. Any required tax reporting forms will be furnished to the appropriate tax authorities.
THE PRIZES ARE AWARDED AS IS AND WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, IMPLIED WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE. SOME JURISDICTIONS MAY NOT ALLOW (OR MAY LIMIT) DISCLAIMERS OF CERTAIN WARRANTIES, IN WHICH CASE THE FOREGOING DISCLAIMERS WILL BE ENFORCED TO THE MAXIMUM EXTENT PERMITTED BY LAW.
The Quiz Promotion Entities are not responsible or liable for: lost, incomplete, inaccurate, garbled, late, delayed, undelivered, or misdirected information or registrations, User ID’s, or submissions; theft, destruction or unauthorized access to, or alteration of, submissions or registrations; failures or malfunctions of connections, cable, fiber-optics, network, satellite, Internet Service Provider (ISP), phones, phone-lines, or telephone systems; interrupted or unavailable network, server, or other connections; any error, omission, interruption, defect, or delay in any transmission or communication; traffic congestion on the Internet or for any technical problem, including but not limited to any injury or damage to participant’s or any other person’s computer related to or resulting from participation in this Quiz; typographical, printing, or other errors in these Official Rules, in any Quiz-related advertisements or other materials; the administration of the Quiz, the processing of entries, selections, or registrations; the selection or announcement of potential Winners or the distribution of Prizes; the cancellation, suspension, or postponement of the 2022 NFL Season; scoring, tabulation, or timing errors; or other problems or errors of any kind whether mechanical, human, electronic, typographical, printing, or otherwise. By submitting an entry into the Quiz, you agree that: (i) Sponsors, NFL, and their respective designees may use (unless prohibited by law) participant’s name, city, and state of residence, photograph and/or likeness for advertising, promotional, trade and/or any other purposes in any media now or hereafter known throughout the world in perpetuity, without further compensation, permission or notification; (ii) Sponsors reserve the right to conduct a background check on winner and entry in the Quiz constitutes participant’s permission for Sponsors to conduct such background check, provided that, to the extent additional authorization is necessary under law, participant shall authorize such check; (iii) Sponsors reserve the right (at its sole and absolute discretion) to disqualify any participant based on the results of the background check or failure of the participant to properly authorize such check.
As a condition of participating in the Quiz, you agree to release, indemnify and hold harmless the Quiz Promotion Entities and their respective affiliates, Prize suppliers, and the Committee’s business partners and all their affiliates, and each of their respective agents, representatives, officers, managers, directors, members, shareholders and employees (the “Released Parties”) from and against any injuries, losses, damages, claims, actions and any liability of any kind resulting from or arising from any breach of representation made by you in connection with the Quiz, the exercise by the Quiz Promotion Entities or their designees of any of the rights granted by you in these Official Rules and your participation in the Quiz, including acceptance, possession, use, misuse or nonuse of the Prize.
As a condition of participating in the Quiz, you agree that as to the Quiz Promotion Entities and the Released Parties (i) under no circumstances will you be permitted to obtain awards for, and you hereby waive all rights to claim, punitive, incidental, consequential, or any other damages, other than for actual out-of-pocket expenses, (ii) all causes of action arising out of or connected with the Quiz, or any Prizes awarded, shall be resolved individually, without resort to any form of class action, and (iii) any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred, but shall in no event include attorneys’ fees.
The Sponsors, in their sole and absolute discretion, reserve the right to: (i) disqualify any person who tampers with the submission process, the operation of Committee’s website, the Quiz platform or any other component of this Quiz, or who is otherwise in violation of these Official Rules; and (ii) modify, suspend or terminate this Quiz promotion (or any portion thereof) for any reason, including should a virus, bugs, non-authorized human intervention, or other causes, including but not limited to war, strikes, and/or acts of God, corrupt or impair the administration, security, fairness, or proper play of this Quiz and, in the event of termination, at its sole and absolute discretion, award Prizes as it deems proper.
CAUTION: ANY ATTEMPT BY A PARTICIPANT TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE QUIZ IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, CLUB RESERVES THE RIGHT TO SEEK DAMAGES AND OTHER REMEDIES (INCLUDING ATTORNEYS’ FEES) FROM ANY SUCH PARTICIPANT TO THE FULLEST EXTENT PERMITTED BY LAW.
SPONSORS’ DECISIONS AND INTERPRETATIONS ON ALL MATTERS RELATING TO THE QUIZ AND THESE OFFICIAL RULES ARE FINAL AND BINDING IN ALL RESPECTS.
By registering in the Quiz, entrants hereby agree to Quiz Administrator’s collection and usage of their personal information.
The potential Winners by acceptance of Prize, except where legally prohibited, also grant permission for Committee and its designees to use his/her name, address (city and state), photograph, voice, and/or other likeness and Prize information for advertising, trade, and promotional purposes without further compensation, in all media now known or hereafter discovered, worldwide, in perpetuity, without notice or review or approval.
YOU FURTHER ACKNOWLEDGE THAT YOUR PERSONAL IDENTIFYING INFORMATION MAY BE DISCLOSED TO THIRD PARTIES INCLUDING, WITHOUT LIMITATION, PLACING YOUR NAME ON A WINNERS’ LIST
Any valuation of the Prize stated above is based on available information provided to Committee, and the value of any Prize awarded to a Winner may be reported for tax purposes as required by law. Winners are solely responsible for reporting and paying any and all applicable taxes. Winners must provide, if required by Committee, at the Committee’s sole and absolute discretion, valid identification and a valid taxpayer identification number or social security number before any Prize will be awarded. Any person winning over $600.00 USD in Prizes from Committee will receive an IRS form 1099 at the end of the calendar year and a copy of such form will be filed with the IRS.
You agree that: (i) any and all disputes, claims and causes of action arising out of or connected with this Quiz or any Prizes awarded shall be resolved individually, without resort to any form of class action, by a state or federal court of competent jurisdiction located in Tempe, Arizona; (ii) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering the Quiz, but in no event attorneys’ fees; and (iii) under no circumstances will participant be permitted to obtain awards for and participant hereby waives all rights to claim, punitive, incidental, and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased.
SOME JURISDICTIONS DO NOT ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES, SO THE ABOVE MAY NOT APPLY TO YOU.
All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules, or the rights and obligations of the participant and the Quiz Promotion Entities in connection with the Quiz, shall be governed by, and construed in accordance with, the laws of the State of Arizona, without giving effect to any choice of law or conflict of law rules (whether of the State of Arizona or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the State of Arizona.
